In order to better serve you, we have provided the following forms to download and complete before your first appointment. We ask that you email your completed forms to frontdesk@gdpeds.com or fax them (623) 878-9150 prior to your visit. 

Please do not use Gel Pens when completing these forms. Use a BLACK Ink Pen only.

Additional Information:

We require a parent or legal guardian to accompany their child for the first visit to our office. Please arrive 10-15 minutes prior to your appointment time.

Walk-in and sibling add-on appointments: Our practice strives to provide timely service along with exceptional customer service. In order to fulfill these obligations we have found it necessary to enforce our policy on walk-in and add on sibling appointments. If you call ahead, we will try our best to accommodate your request.

After-hour appointments: For your convenience we offer after-hours appointments for sick children in the event the regular schedule fills up. These appointment times are determined on a daily basis. The after-hour appointments are considered a “work-in clinic” which means there may be delays. We offer this service to provide continuity of care for our patients.

We require all appointments to be scheduled ahead of time. Non-scheduled requests place the physician’s and staff in a difficult and uncomfortable position. We want to take care of your child’s needs; however, it is unfair to our patients who have a scheduled appointment to wait while someone without a prescheduled appointment is seen.

Late arrivals: Our office offers a grace period up to 15 minutes from the original appointment time. If you are unable to arrive on time for your appointment you will need to contact our office to see if your appointment needs to be rescheduled or work you in behind the scheduled appointments. Well check appointments that are 15 minutes late will need to be rescheduled to another day. Late sick appointments will be rescheduled to the next available opening.

Peak Times: Certain times of the day are “peak” times when the office will most certainly be busy. Such times include “after school” during the school year and all day on Monday during the winter months. If you do not absolutely have to schedule an appointment during these peak times, you would be better served by scheduling your visit during slower periods such as the early afternoon, early morning, and other weekdays.

Certain times of the year are also peak times for certain types of visits. For example, children returning to school need to have their annual well check for sports activities and immunizations. We have found it best to schedule these appointments between April and October.

During the winter months we do experience a high volume of sick children. Some may require immediate attention. This may lead to a delay with your appointment. Our staff will try their best to keep you informed of any delays ahead of time.
