Please note that all teenage well check visits require a teenage questionnaire to be completed at the time of the well check. This questionnaire is used to screen for depression, anxiety, safety, and typical teenage concerns.
Some insurances are applying the charge for this form towards deductibles/co-insurance/co-payments.
We feel this form is necessary to identify any potential concerns that would otherwise be overlooked and as recommended by the American Academy of Pediatrics.
